Service Delivery Description
When a teacher’s request is received the appropriate SERVS Educator is contacted and an initial appointment is scheduled. The initial meeting should be viewed as a pre-conference at which a discussion of the teacher’s requests for support, resources and/or assistance occurs. A plan for delivering the requested support is developed with the teacher and specific activities and dates are established. Once the plan has been implemented a post conference is held to allow the teacher and SERVS Educator the opportunity to review the plan and determine the results of the service delivery. This meeting ends the assignment and following this both the teacher and SERVS Educator are required to complete on-line evaluations.
The SERVS Educator will document all activities on an Activity Log. The log shall document the hours and types of activities engaged in and must be submitted every two weeks according to the FGCU payroll schedule. The Activity Log form is available at the link below. All face to face activities should be initialed by the participating teacher. At the end of each pay period the completed and signed form should be mailed or faxed to the SERVS project office and your hours must also be entered into Gulfline.
